RELEASE STATEMENT

Please read the following release statement.  By clicking "Submit" you fully agree to the conditions mentioned therein.

I hereby release the staff of the Officially-Officials basketball officiating camp, being held in conjunction with the Eckerd College High School Boy's team camp in July 2008, under the direction of Officially-Officials from any and all liability of any injuries or illness incurred while at the school.  Further, I have no knowledge of any physical impairment that would be affected by my participation in the camp program as outlined in the Camp Information.  I also understand that I will not be confirmed for camp unless Officially-Officials receives both my camp fee and a printed copy of my submitted application sent by US Mail.
